- Cleared / not cleared / pending / limited - Which document is missing (physical, waiver, insurance card — whatever you require) - Date the clearance expires if that matters for your program - Staff note for “doctor says no contact until Monday” - Same athlete visible to the coaches who need to know — without giving every volunteer edit rights to everything

Public forms vs internal status

Your **public working site** can collect waivers and releases at Register so parents finish on their phone. That is the door. The **ops desk** tracks whether staff have accepted those documents and whether the athlete may practice or travel. Mixing those jobs — treating a marketing page like a medical database — creates risk and confusion.
